2) Student of Std 1st to 8th have to fill hard copy of from. Form is available at samaj office (Dahisar & Jogeshwari)

3) Applicant should be son/daughter/family member of life time SSPS member.

4) Fields marked with star "*" are mandatory.

5) Last date for online registration: 10-JAN-2012

6) Last date for submitting award nomination form and supporting documents (mark sheets, certificates, etc): 10-JAN-2012

7) Enclose XEROX copy of all necessary supporting documents (mark sheets, certificates, etc) with your application form and submit at samaj office on/before 10-JAN-2012.

8) Enclose RATION CARD copy with your application form.

9) Incomplete form will not be accepted and/or considered. Please verify form's contents before submission.

10) Samaj office timing: 9:00 am - 5:00 pm
